Dr. Nice is back. Risen from the dead. But his experiences have left their mark on him, and everyone doubts whether Nice is fit to work as a doctor again. Nice must learn to deal with the trauma – a trauma he believes he doesn't even have. His rude behavior towards those around him, however, tells a different story. But that doesn't stop Nice from taking on a difficult medical case. When Nice's patient, Mette, shows up at Janne's hotel with her partner, Jesper, an uproar ensues. Because Jesper is the very man who dumped Janne years ago with little Mikkel. Now Mette and Jesper want a child. It's clear to Mette that Jesper can only be the man she truly wants to have a child with if he lives up to his responsibility to Mikkel and finally gets rid of the guilty conscience that is obviously tormenting him. In order to persuade Jesper to take such a step, Mette deliberately chose Nice for her urgently needed breast surgery – so that she and Jesper would "have to" stay at Janne's hotel. There, she hopes, Jesper will inevitably meet Mikkel. The three have a lot to work through in order to put the hurts of the past behind them. During the operation, which is not particularly challenging in itself, however, Mette experiences complications whose cause seems mysterious. Once again, Nice must use all of his medical acumen to get to the bottom of his patient's life-threatening problem. Nice, too, must confront his past and his complicated present, in which doctor's assistant and co-parent Charlie, his daughter Lea, and above all, his favorite enemy, Dr. Schmidtke, repeatedly challenge him and his messed-up genius.
